Old Kage needs to die, he did a nice job combined with Gaara to stop the meteor(s) but that's it for him. He shouldn't match up to Madara at all.
Madara looking down his chest? Maybe he has some special heart or something, in any case he definitely has the "Body" or Senju to be able to use wood techniques and to make that sick meteor.
Tsunade going out? Bet she either has some peach army healing/saving roll against one of Madara's attacks or she fights to a scrub extent.

Only reason he'd do it himself is Naruto being busy fighting Tobi (doubt it, Tobi rarely fights) or goes on to find Sasuke (doubt it, Naruto's goal is "Madara" to end war).
