GMS, JMS and KMS all seem to be big offenders in that area if so. Makes me wonder though, which was the very first version to take content from another version and change it to suit their needs?

Both (but mostly the former). I think this is the first time I've seen GMS alter KMS content to this extent.

Wouldn't the first have been either JMS' dual blade or EMS getting Cygnus before Pirates? (If that counts)

I would say both those count. But since JMS did the Dual Blade change first, right? I know EMS is slow to update and everything, but I am not sure. I guess that would make JMS the very first server to alter content for their own vision.

I'm not sure how accurate it actually is, but the timeline says that EMS got the Cygnus Knights in 2009 while JMS got Dual Blade sometime in 2010.

Hmm. Still, like Scott said getting something before you're supposed to doesn't really count, like with Kasier. JMS was the first to actually change content in a serious way from another version.
